Soft tissue injuries are a typically presenting injury in people who undertake increased exercise such as runners.  These may well occur in the foot and ankle and when managing this patient group you have to consider their lifestyle and needs as often maintaining their fitness is very important to them.
Consider the ankle and rearfoot areas.  By considering the regions medially, laterally, anteriorly and posteriorly, list the soft tissue structures that are commonly injured.

For each area, identify how you would recognise that one of these structures are injured.  For example, if thought the Achilles tendon was affected, how would you recognise that it was injured?

Injuries are often described as being acute or chronic.  Define when an injury is acute or when it is chronic.  In general the acute / chronic injury is determined by healing time but occasional it is determine by the length of time the injury is occurring over.  You may wish to consider this when forming your definition.  Another term sometimes used for injury is “acute-on-chronic” – what does this mean?

If a patient had an eight week history of symptoms affecting a tendon, would you consider this acute or chronic?

The treatment for an acute injury would differ from that of a chronic injury.  Taking a tendinopathy as an example, describe your initial management of an acute tendinopathy and compare this to the management of a chronic tendinopathy.  Consider the time scales in your management.  For example, if you are suggesting ice is used – at what point are you suggesting its use, for how long, who often and how is it applied.  The initial management is usually very generalised – ie. it would apply to any injured structure.  What is the theory behind the use of the treatment you are applying?  For example how does ice work?

The mid and longer term management of an injury becomes much more focused on the specific structure involved and the possible cause of that injury.

What is meant by intrinsic and extrinsic etiological factors of sports injuries?

Consider the example for a tendinopathy in the foot, for example an Achilles tendinopathy, what would be the possible intrinsic and extrinsic causes?

How would you address these intrinsic and extrinsic factors in the mid and longer term management of your patient? Consider the example of the Achilles tendinopathy and describe how you would manage the patient. Again be precise about the time lines, when / how long for.  And be precise about the treatment given – why (theory as to what you are using this treatment for) / how (do you apply the treatment)?

Finally, given that most sports people want to keep active.  What considerations do you need to make when managing injuries in this particular patient group?
