Patient is 59 years old came by himself to A&E at 12:50 complaining of chest pain, the pain started 2 days ago while he was playing football. He thought the pain was because of gastric gases and he took two tablets of Paracetamol. Patient running his own business as art designer and he was well and healthy. Also he has no past medical history and he was not smoker.

The vital signs is:
BP: 139/79
SO2: 95%
GCS: 15
HP: 90
RR: 20
Temp: 37
ECG: Abnormal

It?s worth to note that his father died when he was 58 years old because of Myocardial infarction before 30 years.
Patient is in upright position; nurse took blood sample at 12:55.
She gave him 300 mg of Aspirin orally at 13:00 with 60 mg of Efient and Nitrolingual 2 puffs (800 microgram).
At 13:40 the patient transferred to another hospital by ambulance.

In this case I will discuss how the Myocardial infarction could be hereditary.
